Manager / Coach Evaluation Form
The West Islip Little League would like your input in the evaluations of our managers
and coaches. As the parents/guardians of our players, your thoughts are of great
importance in choosing the most qualified managers and coaches for the future seasons. 
Each form is kept strictly confidential and is never seen by anyone other than a select
few members of the W.I.L.L. board. The manager/coach
WILL NEVER see your evaluation, so please, be as candid as possible.
